OBS Studio Intermittently Crashes While Streaming (d3d11.dll Error)

Hi There!

Over the past couple weeks, I've been having intermittent OBS crashes while streaming to Twitch. In each instance, the crash reports indicate the same issue related to d3d11.dll.
In troubleshooting last week, I did a clean install of my NVidia drivers as well as an SFC /scannow repair from Command Prompt. In that case, Windows found corrupted files, but was able to repair. The issue seemed resolved over the last 3 days with no issue, but today the problem returned with 3 crashes during my broadcast.
I've attached the most recent crash/log files for review. Any ideas as to what may be causing this & next steps? Thanks for your time!

You're using SE.live plugin. Please contact them for support.
Alternatively, uninstall it. Then uninstall and reinstall OBS in order to fix the changes made by this plugin. Now please do a test stream/recording of more than 30 seconds and post the log file here.
 
This evening I did another clean Nvidia driver install, verified that no issues were present again in command prompt (SFC /scannow), and uninstalled my previous version of OBS with SE.Live. Following a PC restart, I did a clean OBS Studio install (with restored scenes/settings from backup - with no plugins) without SE.Live. I also added a closed captioning & waveform plugins to the clean install. I restarted the PC again. After that, I ran a test stream for appx. 1.5 hours switching between scenes & capturing gameplay without issue. I've attached the log from this test. Hopefully the uninstall/reinstall resolved my issue. I'll update if the issue returns. Thanks again!

Also, check the plugins if this are up to date and compatible with latest version of OBS. Remove any plugin you're not using.
